DUPLICATE FIREARMS IDENTIFICATION CARD

Fill out Application (STS-3) (PRINT TWO COPIES & SIGN BOTH COPIES) and bring to the Police Department, you will need to acquire a Criminal History Record Information Form (Not available on the N.J.S.P. website).

STS-3 Duplicate Identification Card (change of address – damaged card - name change)

Cost: $18.00 Money Order payable to Division of State Police/S.B.I. for Criminal History Check
Cost: $5.00 for replacement ID Card (Cash or check payable to: Delaware Township Police)

Applications are accepted Monday - Friday between the hours of 8:00 A.M. – 3:00 P.M during regular business hours. Applications will not be accepted by officers.
