PRINT TWO COPIES AND SIGN EACH COPY

1. CONSENT FORM– Fill out, date and sign.  If this does not apply to you just date and sign.
   
   
2. DOMESTIC VIOLENCE SHEET – Fill in completely.  If a line does not apply to you fill in N/A.  Sign and date.
   
   
3. FIREARMS APPLICANT QUESTIONNAIRE – Fill out completely.  Place N/A in the blocks where questions do not apply to you.  Complete both sides.  Date Application.
   
   
4. JUVENILE RECORDS SEARCH – Fill out completely.  Sign Form.  DO NOT HAVE ANYONE SIGN THE WITNESS LINE.  TO BE SIGNED BY THE POLICE DEPT.  Date the Form.
   
   
5. MEDICAL RELEASE AUTHORIZATION – Fill in you name next to I.  Date and Sign Form.  DO NOT HAVE ANYONE SIGN THE WITNESS LINE.  TO BE SIGNED BY THE POLICE DEPT.
   
   
6. MENTAL HEALTH RECORDS SEARCH – Fill out ONLY THE HIGHLIGHTED AREAS.  DO NOT HAVE ANYONE SIGN THE WITNESS LINE.  TO BE SIGNED BY THE POLICE DEPT.    • Mental Health http://www.state.nj.us/njsp/info/pdf/firearms/sp-066.pdf
   When all paperwork is completed, obtain a certified check or money order for $60.25 payable to
   “DIVISION OF STATE POLICE – SBI”  ABSOLUTELY NO PERSONAL CHECKS WILL BE ACCEPTED.
   
   
7. FINGERPRINTING is done by appointments only. Fingerprinting is done Monday, Wednesday and Friday 8:00 PM til 8:30 PM.  Call 609-397-8189 Monday – Friday 8:00 a.m. til 3:30 p.m. to make an appointment.  Bring all paperwork with you when you get fingerprinted.

There is a $5.00 charge for the ID Card,    $2.00 for Each Handgun Permit
